TERMS & CONDITIONS

I hereby grant my child permission to participate voluntarily in the Evolve Together Basketball Academy Workouts. I acknowledge and accept that sports involve inherent risks, and injuries are common occurrences. I willingly assume all risks associated with my child's participation, including the risk of injury or death. In exchange for allowing my child to participate, I release and hold harmless Evolve Together Basketball Academy, its staff, volunteers, and designated coaches from any liability, including any claims for injury or damage to my child, other individuals, or property. This release applies to negligence or other acts by employees or volunteers. I agree that this release is binding on me, my child, and our heirs and assigns.

Additionally, I agree to indemnify and hold Evolve Together Basketball Academy, including its employees and volunteers, harmless from any loss, liability, damage, cost, or expense resulting from any injury or property damage caused by my child during participation

Media Release Form

I hereby authorize Evolve Together Basketball Academy and its staff to use photographic portraits, pictures, or digital images of my child, in which my child may be included, either in whole or in part. I grant this permission for any lawful purpose, including but not limited to use in sports publications, social media, or on the website of Evolve Together Basketball Academy, without any payment or consideration. I waive any right I may have to inspect or approve the finished product or its associated copy wherein my child’s likeness appears, or the manner in which it may be used.
